Amendment #1106 to H4000

The Yellow Dot Program

Representatives Durant of Spencer, Kuros of Uxbridge, Frost of Auburn, Dooley of Norfolk and Fattman of Sutton move to amend the bill in section 2 by inserting after item xxxx-xxxx the following item:

 

"xxxx-xxxx For the operation of an Emergency Responder Yellow Dot Program (yellow dot) to be used on automobiles in the Commonwealth which shall provide emergency responders with critical health and emergency contact information about program participants so they may aid participants who are involved in motor vehicle emergencies and are unable to communicate; provided the program includes a standard medical information form providing space for each participant to supply, at a minimum, the following information:  name; a photograph of the program participant taken within the past 5 years clearly showing the participant’s face; emergency contact information of no more than 2 persons; medical information, including medical conditions, recent surgeries, allergies, medications being taken, specific medical instructions and any religious objection to receiving treatment; hospital preference; no more than 2 physicians' contact information; and the date on which the participant completed the form; provided further, that the medical information form shall include a statement that the yellow dot program acts as a facilitator only, and that all information supplied on the medical information form is the sole responsibility of the program participant and that the participant authorizes the disclosure to, and use of, the medical information by emergency medical responders and other responders for the purposes described in this section; provided further, that such a card be placed in the glove compartment of a vehicle in an easily seen yellow envelope and be accompanied by a uniform sticker decal to be placed on the bottom driver’s side corner of the rear windshield of a vehicle for uniform identification; provided further, that individual participation in said program is voluntary,      $1,000,000"
