Continuing Care Management

Mr. Richard T. Moore moves to amend the bill in section 1, by striking out, in line 23, the words “enabling transitions from more intensive to less intensive treatment” and inserting in place thereof the following words:- “enabling successful transitions from more intensive to less intensive treatment, based on the medical needs of the patient”; and

in said section 1, by striking out, in line 49, the words “from time to time,” and inserting in place thereof the following words:- “upon application for recertification and at other times as required by the commission”.
