Caring Health Center

Mr. Swan of Springfield moves that the bill be amended in section 2, line item 4510-0110, by adding the following: “provided further, that not less than $400,000 shall be expended to the Caring Health Center/ Richard E. Neal Complex Community Health Center in Springfield to transform its Wellness Center into a Patient Centered Medical Home (PCMH)providing chronic disease prevention and management for the medically underserved promoting modifiable behavior changes that translate to patient improved health.”
