Social Determinants of Health

Mr. González of Springfield moves to amend the bill in section 2 in line item 4000-0300 by adding at the end the following: "And provided further the Executive Office of Health and Human Services shall make available to a Western Massachusetts Academic Medical Center with three Community Health Centers no less than $375,000 to be matched by said Western Massachusetts Academic Medical Center for the purpose of developing Infrastructure connecting the greater Springfield community to healthcare.  Said program shall install cloud-based software to support collaborative care coordination for the patient and the team caring for the patient including but not limited to community health workers, family, food banks, housing agency staff, transportation agencies and healthcare providers.  Said program shall implement a unified entry and case management strategy across the community; standardize screening, assessments and care coordination; share data between healthcare providers, ACOs, community partners, PVIX and public systems; and integrate workflows across many agencies, health plans and providers in the Pioneer Valley"
