Hospital to Home Partnership Program

Mr. Cahill of Lynn moves to amend the bill in section 2 by inserting after item 9110-0100 the following line item:-

XXXX-XXXX For the expansion of the Hospital to Home Partnership Program to promote partnerships between hospitals and Aging Services Access Points (ASAPs) and to strengthen communication and coordination with community providers to promote institutional diversion and increase discharge rates from hospital to home ………………. $1,500,000
