Complex Care Ombudsman Program

Mr. Driscoll of Milton moves to amend the bill in section 2, in item 4000-0300, by adding the following: “provided further, that not less than $550,000 shall be expended for a complex care ombudsman program within the executive office to assist acute & post-acute care hospitals with discharges to lower level post-acute care settings; provided further, that such program shall ensure that at least one complex care case manager shall be assigned to each of the 5 EMS regions of the state to assist the hospitals in each region with discharges to lower level post-acute care settings”.
